An employee was fatally injured after being struck by part of the paper machine breast roll.

AusJet advises that the risks associated with using the "whip check" type hose restraints of either steel or nylong manufacture as a safety device to secure hose connections are unacceptable and it is recommended that only the steel or stainless stell "stocking" type of restraint be used in all instances.
